Semaglutide vs. tirzepatide: what head-to-head data shows

SURMOUNT-5 and real-world cohorts consistently favor the dual agonist on weight endpoints — with nuances the headlines skip.

The head-to-head question is now answered at trial scale: SURMOUNT-5 reported significantly greater weight reduction with tirzepatide than semaglutide at matched treatment intent. Real-world cohort data has landed in the same direction.

The nuances matter. Discontinuation rates, tolerability profiles, and cost/access considerations narrow the practical gap, and semaglutide’s cardiovascular-outcomes evidence base remains deeper. For diabetes-specific endpoints the comparison is closer than for weight.
