Three PT-141 specimens from three different companies, tested across 2025–2026, all measured between 14% and 19% below label content while passing purity screens. Independent single failures are unremarkable; a compound-specific pattern across suppliers points upstream.
The most plausible explanation is shared sourcing: bulk PT-141 entering the supply chain pre-shorted, with downstream brands filling vials by nominal weight without verification. It is exactly the failure mode that batch-specific quantity testing catches and standard purity COAs miss.
We have added PT-141 to our recurring verification rotation and notified the affected companies of our findings.
